I hereby consent to the processing of my personal data by Ebisu Studio Michał Jakubek with the tax identification number (NIP) 898-204-86-55 for the purpose of contacting the organizer of the JAPFEST event, in accordance with the Personal Data Protection Act of August 29, 1997 (Journal of Laws of 1997, No. 133, item 883) and Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR). The data will be processed for one year. The consent is voluntary. I am aware that I have the right to access my data, rectify and delete them, as well as request restriction of their processing due to my particular situation, and withdraw the consent given at any time, whereby the withdrawal of consent shall not affect the lawfulness of processing carried out before its withdrawal, and also lodge a complaint with the supervisory authority. The data will not be transferred to third countries and will not be subject to profiling, i.e., automatic decision-making.
